IL Department of Insurance (IDOI) – Standardized Health Insurance Applications

As part of the Illinois Insurance Fairness Act, the Illinois Department of Insurance has developed a standardized small group health insurance application to make it easier for us to present you to various carriers.  All health insurance carriers are required to start using the standard application in place of their current applications beginning February. 1, 2011. The new applications will be used for new business as well as existing business and renewals.  The old applications will no longer be accepted after January 31, 2011.
